In this form, you can request an invoice for a completed trip.

How do I request an invoice?

• If you have an active reservation, you can generate an invoice in the Manage my booking (在新窗口中打开) tab.
• If your trip has already been completed, fill out the form below.
• Need to receive an invoice for a reservation purchased through a travel agent? Contact the ticket issuer directly.

How much time do I have to request an invoice (from 2 January 2026)?

• Private individual (no tax ID):
  You have 3 months from the end of the month in which the flight took place (in line with VAT regulations).
  Example: if your flight took place on any day in February, you can request an invoice until 31 May.
  
  
• Company (tax ID provided):
  Please submit your request before the 15th day of the month following the month in which the flight took place, so we can issue the invoice within the legal deadline.

When will I receive the invoice (from 2 January 2026)?

• Private individual (no tax ID):
  • If you request the invoice in the same month as the flight, you will receive it by the 15th day of the following month.
  • If you submit the request later (but still within the 3-month limit), you will receive the invoice within 15 days of the request date.
    
    
• Company (tax ID provided):
  • You will receive the invoice by the 15th day of the month following the month in which the flight took place.

The controller of personal data is LOT Polish Airlines S.A. with its registered seat at ul. Komitetu Obrony Robotników 43, 02-146 Warsaw. Your data will be processed exclusively for the purposes related to issue of a VAT invoice. You have the right to access and correct the content of your data. Provision of personal data is voluntary, however indispensable to issue a VAT invoice in accordance with the Decree of the Minister of Finance of 28 March 2011 regarding reimbursement of tax to certain groups of taxpayers, issuance of invoices and manner of their storage and a list of goods and services which cannot enjoy tax exemptions (Journal of Laws of 2011 No. 68, item 360). I hereby accept electronic issue and transmission of VAT invoices (PDF format) to the abovementioned address by LOT Polish Airlines S.A., in conjunction with the Ordinance of the Minister of Finance of 17 December 2010 on electronic transmission of invoices, rules of their storage and presentation to the tax or fiscal audit authorities (Journal of Laws No. 249, item 1661).
