How do I receive an invoice for an agency booking?

We kindly inform you that we are unable to process your request. It is possible to request an invoice for tickets issued on our website www.lot.com, Call Center or LOT Polish Airlines offices. To obtain an invoice for a ticket purchased at a travel agency, please contact the ticket seller directly.

How do I get an invoice for purchased insurance?

It is not possible to issue a VAT invoice for an insurance policy. According to par. 3 para. 2 of the Decree of the Minister of Finance dated December 3, 2013. Journal of Laws. 2013 item 1485 on the issuance of invoices, the insurance policy itself is already an invoice. The request to resend the policy should be addressed directly to the insurer. How to change the data on the invoice?

If you change one element in the data on the invoice, it is necessary to send a signed correction note by e-mail in pdf format or by regular mail to: LOT Polish Airlines 43 02-146 Komitet Obrony Robotników Street, Warsaw. If there are more elements to be changed, please fill out the form below.

Your data will be processed exclusively for the purposes related to issue of a VAT invoice. You have the right to access and correct the content of your data. Provision of personal data is voluntary, however indispensable to issue a VAT invoice in accordance with the Decree of the Minister of Finance of 28 March 2011 regarding reimbursement of tax to certain groups of taxpayers, issuance of invoices and manner of their storage and a list of goods and services which cannot enjoy tax exemptions (Journal of Laws of 2011 No. 68, item 360). I hereby accept electronic issue and transmission of VAT invoices (PDF format) to the abovementioned address by LOT Polish Airlines S.A., in conjunction with the Ordinance of the Minister of Finance of 17 December 2010 on electronic transmission of invoices, rules of their storage and presentation to the tax or fiscal audit authorities (Journal of Laws No. 249, item 1661).
